SponsorshipApplication Form" andthe "SponsorshipIntention and DonationMethod Survey Form",please download and print them yourself.(If the student is currently sponsored by the foundation,please do not apply again)III.Please be sure to fill out the application form according to the "Instructions for Filling" in the application form, and accurately fill in the mobile phone numbers and email addresses of the student and parents,and be sure to prepare all relevant documents.* Copy of student's household registration transcript or household registration book1 copy* 2-inch photo1 photo(Write the school and name on the back, and clip it to the upper left corner of this application form with a paperclip)
* Copy of savings passbook cover* Copy of low-income or middle-low-income household certificate(If you do not have one, please apply for the most recent year's individual income statement for the student's parents or main caregiver from the National Taxation Bureau)After completing the form, please submit it to the school coordinator (Deputy Section Chief of the Counseling Office) for review by 03/17 (Mon).

填表完成後，請於03/17(一)前繳交學校承辦人(輔導室副組長)檢核 。
